The RGDU has developed a number of online request forms to assist with the implementation of key processes and procedures. RGDU online forms allow authorised users and stakeholders to input information regarding Health Protection Guideline Requests, Evidence Review Requests, and Requests for Updates to RGDU Guidance Documents.

RGDUF005 Registration Form for National Health Protection Office (NHPO) Research Projects

This form collects key details of your research project to ensure organisational oversight, compliance with governance and ethics requirements, and accurate reporting.

RGDUF007 Out of Hours Handbook – Feedback Submission Form

Please complete the following form, if you have suggested changes or amendments to the Out of Hours Handbook. This form should be authorised by a Consultant in Public Health Medicine (CPHM). Click on the link in title above to complete.

RGDUF009 Stakeholder Implementation Form

The purpose of this form is to gather information from core stakeholders on the acceptability and usability of newly published guidance or guidelines. All guideline development group members are requested to complete this form
